Morphine sulphate
Indications/Uses
Symptomatic relief of moderate to severe pain especially associated w/ neoplastic disease, MI & surgery. Adjunctive therapy in acute pulmonary oedema secondary to left ventricular failure.
Dosage/Direction for Use
Analgesic Adult IM or SC Initially 10 mg every 4 hr as needed. Dose range: 5-20 mg. Slow IV 4-10 mg diluted in sterile water for inj 4-5 mL. Epidural 5 mg w/ incremental doses of 1-2 mg/hr if needed up to max: 10 mg/24 hr. Intrathecal 0.2-1 mg/kg as single dose. Paed SC 0.1-0.2 mg/kg every 4 hr as needed. Max: Not to exceed 15 mg/dose. Very slow IV 0.05-0.1 mg/kg. Pre-op 0.05-0.1 mg/kg IM. Max: Not to exceed 10 mg/dose.
Contraindications
Resp depression w/ cyanosis & excessive bronchial secretion; biliary tract post-op; acute alcoholism; head injuries & conditions increasing ICP; hypotension, asthma. History of drug abuse. Concurrent MAOI therapy; administration of narcotic analgesics. Hepatic impairment.
Special Precautions
Decreased resp reserve; myasthenia gravis; hypothyroidism, adrenocortical insufficiency, prostatic hypertrophy, shock, inflammatory or obstructive bowel disorders. May affect ability to drive & use machines. Renal or hepatic impairment. Pregnancy. Not recommended during lactation. Not to be given pre-op in childn <1 yr. Newborn or premature infants. Elderly & debilitated patients.
Adverse Reactions
Nausea, vomiting, constipation, drowsiness; resp depression & hypotension (large doses); micturition difficulty, ureteric or biliary spasm, dry mouth, sweating, facial flushing, vertigo, bradycardia, palpitations, postural hypotension, hypothermia, hallucinations, mood changes, dependence, miosis, urticaria, pruritus.
Drug Interactions
Enhanced depressant effects w/ CNS depressants eg, alcohol, anaesth, hypnotics & sedatives, TCAs, phenothiazines. Precipitated w/drawal symptoms w/ opioid analgesics eg, buprenorphine, butorphenol, nalbuphine or pentazocine. Delayed mexiletine absorption. Counteracted metoclopramide effects.
